Arthritis in Dogs - How to Care For Canine Arthritis


Arthritis in dogs is a painful, progressive and degenerative disease causing inflammation of a joint which can inhibit movement. More properly called osteoarthritis, although there are other types of arthritis, this is the most common, the most easily treated and is the focus of this article.

The cause of canine arthritis in most cases is the simple wear and tear to bones and joints over time. Arthritis often occurs at a point of pervious injury or when there is a history of infection such as lyme disease.

Osteoarthritis mainly occurs in older dogs over the age of 7 years and is more widely seen in larger breeds, such a German Shepherds, Labradors and Golden Retrievers. Overweight dogs are also more likely to suffer from arthritis. In young dogs it may be caused by failure of proper bone development.

Arthritis in dogs is frequently accompanied by hip dysplasia. Together these two ailments can seriously impair your dog's mobility and quality of life.

Symptoms are subtle to start with such as decreased interest in activity or lagging behind on walks and progress to more obvious ones:


  • difficulty getting up and down

  • reluctance to run, jump, or climb stairs

  • stiff, sore or swollen joints

  • limping

  • sensitivity to cold

  • behavioral changes such as aggression, withdrawal or irritability


Treatment - while there is no way to reverse the problem at present, arthritis can be treated with medications or less often, hip-replacement and other types of joint surgery. Most dogs will improve with anti-inflammatory medications which can reduce the pain and swelling of damaged joints caused by osteoarthritis and increase mobility. Be aware; however, of possible side effects involving the digestive system, kidneys, or liver.

When it comes to treatment of arthritis in dogs, many pet owners and veterinarians take an integrative approach, combining prescription medications such as non-steroidal anti-inflammatory drugs with supplements and alternative treatments such as acupuncture. Some of the more popular and sometimes effective supplements are glucosamine, chondroitin, msm, ester-c, and hyaluronic acid. Many dog owners have reported positive outcomes using these natural supplements.

Weight loss can help relieve stress on joints. Older large-breed dogs tend to be overweight. This discourages them from getting the exercise they need and may aggravate their arthritis. The more weight on those poor old joints, the harder it is to move around.

Prevention is one of the kindest and most effective measures you can take. Keeping your dog in shape through regular exercise and proper nutrition for optimal health is your best line of defense against this painful disease, as well as your best approach after your dog has been diagnosed. Another key is to maintain proper weight and limit snacks and foods that can aggravate inflammation.

Much can be done to make arthritic dogs more comfortable and improve their lives.


  • prevent stressing weak painful joints or falling down - place gates across stairs

  • elevate food and water dishes

  • portable ramps or steps - for home and vehicle

  • rubber place mat to stabilize food and water dishes that slip on bare floors while your dog is eating

  • cushion joints - orthopedic dog beds at least four inches thick

The bottom line is arthritis in dogs needs veterinary care. If you suspect your best buddy has arthritis, follow the advice of your veterinarian.

Senior Citizens Rheumatoid Arthritis, Osteoarthritis, and Arthritis - Causes and Treatments


"Arthritis" does not mean only that someone has stiff, aching joints. Many types of arthritis exist, each with its own symptoms and treatments. Most types are chronic, meaning that they can be a source of discomfort for an extended period of time. Arthritis can afflict joints almost anywhere in the body and may cause changes you can see and feel, including swelling, warmth, and redness in the joints. It can last for a short time but be very painful or continue for a long time with less pronounced results while still damaging the joints.

Arthritis is extremely common in the United States, especially among senior citizens. Still, there are many steps they and those providing care for the elderly can take to relieve the different types of arthritis. The most common types in this population are osteoarthritis, rheumatoid arthritis, and gout.

Osteoarthritis.

Osteoarthritis (OA) is the most common form of arthritis in senior citizens and begins when cartilage, the type of tissue that pads joints, begins to wear away. This can eventually cause all the cartilage between bones to wear away, forming painful rubbing of bones against each other. This type of arthritis is most common in the hands, neck, lower back, knees, and hips.

Symptoms of OA can range from stiffness and mild pain that accompanies exercise or bending to severe pain in the joints even in times of physical rest. OA can also cause stiffness during times in which you haven't used specific joints in a while, like when you're on a long car ride, but this stiffness usually goes away when you move your joints again. OA can eventually lead to problems moving joints and sometimes to developing a disability if the areas affected are the back, knees, or hips.

Aging is often the greatest risk factor for developing OA. Other factors depend on the area of the body afflicted-for instance, OA in the hands or hips may be caused by genetic factors; OA in the knees may be caused by being overweight; and injuries or overuse of joints in the knees, hips, and hands may lead to OA.

Rheumatoid arthritis.

Rheumatoid Arthritis (RA) differs from OA in that it's an autoimmune disease, meaning that your immune system attacks and damages the lining of a joint as if it were an injury or disease. RA leads to inflammation of the joints, which causes pain, stiffness and swelling, sometimes in multiple joints at once. It may be severe enough to prevent you from moving a certain joint. Senior citizens with RA may often experience fatigue or fever. You can develop RA at any age, and it's more common in women. 

RA can afflict almost any joint in the body and is often symmetrical, meaning that if you have RA in a specific joint on one side of your body, you probably experience RA in the same joint on the other side of your body. RA can damage not only joints, but also the heart, muscles, blood vessels, nervous system, and eyes.

Gout.

Senior citizens with gout experience the most severe pain relative to many other arthritis patients. An attack begins when uric acid crystals form in the connective tissue or joint spaces, leading to swelling, stiffness, redness, heat, and pain in the joint. Attacks often follow eating foods like shellfish, liver, dried beans, peas, anchovies, or gravy. Drinking alcohol, being overweight, and taking certain medications may worsen the symptoms. In senior citizens, using certain medications to lower blood pressure may also be a risk factor for a gout attack.

Gout is most common in the big toe, but it can occur in other joints such as the ankle, elbow, knee, wrist, hand, or other toes. Swelling may cause discoloration and tenderness due to skin stretching tightly around the joint. If you see a doctor during an attack, he or she may take a sample of fluid from the affected joint.

Other forms of arthritis.

Other forms include psoriatic arthritis  in patients who have psoriasis; ankylosing spondylitis, which mainly affects the spine; reactive arthritis, which occurs as a reaction to another illness in the body; and arthritis in the temporomandibular joint, the point at which the jaw attaches to the skull.

Arthritis Symptoms and Warning Signs.

Senior citizens and those providing their elder care should look out for the following symptoms as they may be indications of arthritis:


  • lasting joint pain

  • swelling in a joint

  • stiffness in a joint

  • tenderness or pain when touching a joint

  • difficulty in using or moving a joint normally

  • warmth and redness in a joint

Any of these symptoms lasting longer than two weeks should be addressed by a physician. If you experience a fever, feel physically ill, have a suddenly swollen joint, or have problems using a joint, a doctor should be contacted sooner. You will have to answer questions and go through a physical exam. Before suggesting treatment options, your doctor may want to run lab tests and take X-rays.

Arthritis Treatment.

Some common treatment options exist even though each type of arthritis is treatedsomewhat differently. Rest, exercise, eating a healthy diet, and becoming educated about the right way to use and protect the joints are key to minimizing the effects of arthritis. Proper shoes and a cane can minimize pain the feet, knees, and hips while walking, and some technology exists for helping open jars or bottles, turn doorknobs more easily, and otherwise improve quality of life in senior citizens with arthritis. 

Additionally, some medications can lower the pain and swelling. Acetaminophen (in Tylenol) and some NSAIDs are sold over-the-counter and can ease pain. Other NSAIDs must be prescribed. It is important for senior citizens and those providing their in home care to pay attention to the warnings on both prescribed and over-the-counter drugs and to ask a doctor about how to properly and best use over-the-counter medicine to treat arthritis. The FDA also has information about many medications.

Some treatment options are specialized for individual types of arthritis.

Osteoarthritis Treatment.

There are medicines to help senior citizens with pain associated with OA, and rest and exercise may ease movement in the joints. Managing weight is also important. If one experiences OA in the knees, a doctor can provide shots in the knee joint, which can help to move it without as much pain. Surgery may also be an option to repair or replace damaged joints in senior citizens.

Rheumatoid Arthritis Treatments.

Treatment can diminish the pain and swelling associated with RA and cause joint damage to slow down or stop. One will feel better overall, and it will be easier to move around. On top of pain and anti-inflammatory medications, a doctor might prescribe DMARDs, which are anti-rheumatic drugs that can slow damage from RA. Corticosteroids, including prednisone, can minimize swelling while waiting for DMARDs to kick in. Additionally, biogenic response modifiers block the damage inflicted by the immune system and help people with mild to moderate RA when other treatments have failed to work properly.

Gout Treatment.

If you've gone through a gout attack, talk to a doctor to discuss possible causes and future prevention of attacks. Work together with your doctor and other elder care providers to plan and execute a plan for prevention. Commonly, NSAIDs or corticosteroids are recommended for an acute attack. This treatment diminishes swelling, allowing you to feel better fairly shortly after treatment. Usually, the attack fully stops within a few days. If one has experienced multiple attacks, a doctor may be able to prescribe medication to prevent further attacks.

Exercise can help Arthritis.

In addition to taking the proper medication and allowing your joints to rest, exercise can help senior citizens to stay in shape, maintain strong muscles, and control symptoms of arthritis. Daily exercise like walking or swimming keeps joints moving while lessening pain and strengthening the muscles around joints. Before starting any new exercise program, it is important to discuss options with your physician. 

Three types of exercise are the best for senior citizens with arthritis:


  • Range-of-motion exercises reduce stiffness, improve flexibility, and keep joints moving. Activities like dancing fit into this category.

  • Strengthening exercises strengthen muscles, which improves support and protection to your joints. Weight training fits into this category.

  • Aerobic or endurance exercises improve health in the heart and arteries, prevent weight gain, improve how your body works overall, and may decrease swelling in some joints. Riding a bike fits into this category.


Other things to do to manage Arthritis.

On top of exercise and weight control, a number of other methods may help senior citizens ease the pain around joints. Applying heat or cold to joints, soaking in a warm tub, or swimming in a heated pool may help you feel better and move your joints more easily.

Surgery may be an option when damage has become disabling or when other treatment options have not adequately diminished pain. With surgery, joints can be repaired or replaced with artificial ones. Commonly, arthritic knees and hips are replaced.

Unproven remedies.

Many senior citizens with arthritis try treatments that have not been tested or proven to help. Some are harmful, like snake venom, while others are harmless yet unhelpful, like copper bracelets.

Here are a few ways to determine whether a treatment is unproven:


  • The remedy is said to work for all types of arthritis and other diseases

  • Scientific support is from only one research study

  • The label doesn't include directions or warnings of use


Areas for further research.

Studies suggest that acupuncture could ease OA pain in some senior citizens. Dietary supplements such as glucosamine and chondroitin are also under investigation and may reduce OA pain. More research is needed to determine whether these types of treatments actually work to reduce symptoms and damage to joints.

Talk to your doctor and others involved in your elder care.

Try not to make light of your symptoms by telling yourself that joint pain or stiffness is simply caused by aging normally. Your doctor and other elder care providers can discuss possible treatment options with you to safely minimize your pain and stiffness and prevent more serious joint damage.

Could Temporomandibular Disorders Be Causing Your Jaw Joint Pain And Headaches?


Joint pain and headaches can be caused by countless things, including diseases such as arthritis, but obviously I can't cover each individual one in a single article. Seeing that I've already written several articles about different types of arthritis, I'll focus on TMJ (The Temporomandibular Joint) disorders instead this time.

As with other similar disorders, TMJ disorders can be difficult to diagnose, and equally as difficult to treat. A patient suffering from one of these disorders often has to undergo several tests, including things like MRI scans and etc.

So, what is the temporomandular joint? The temporomandular joint is a ball and socket type joint situated on either side of your face. They are the joints which connect your upper and lower jaws to each other, and are considered being the most used joints in the human body. Apart from all the regular movement, such as when you open or close your mouth, the joints are also subjected to a great deal of pressure whenever we chew our food.

Supporting these joints are a complex arrangement of muscles, ligaments, and bones, so when something goes wrong with one or both joints, it's quite often accompanied by muscle pain and headaches.

TMJ disorders can, as previously mentioned, be caused by several things. With that having been said, the most common causes are physical trauma, teeth grinding, gum chewing, and of course misaligned teeth.

People with one or more TMJ disorders often feel a clicking sensation when they open their mouth, and particularly when they yawn. They can also have difficulty chewing, or biting down on hard foods. Many sufferers also end up with regular headaches, and approximately 33% of sufferers experience tinnitus, in either one or both ears. Fortunately, surgery is usually only necessary in extreme cases, such as when ligaments need to be tightened.

In the vast majority of cases, patients are advised to rest their jaws as much as possible. This includes eating soft foods as opposed to hard crunchy foods. The next form of treatment a doctor is likely to recommend is heat and ice therapy. Doctors may also recommend that you take anti-inflammatory medication. Of course since I prefer to stick with all-natural remedies whenever possible, I'd most likely opt for a natural anti-inflammatory such as New Zealand green lipped mussel extract.

If teeth grinding is thought to be the cause, a patient might be instructed to use a special type of mouth guard designed specifically to stop one from grinding one's teeth. On the other hand, if misaligned teeth are the root cause, dental work might be necessary.

As mentioned earlier, there are several health problems which can lead to joint pain and headaches, so if this is something you're experiencing, then it is best that you see a doctor so that a proper diagnosis can be made.

Who Else Wants General Information on Arthritis?


Today there are about 70 million Americans with arthritis...that's one person out of every four who suffer both pain and the expense of this crippling disease. In one year alone, arthritis will be responsible for over half a billion dollars in lost wages. The economic consequences of arthritis are important to review because each year, arthritis takes a devastating financial toll on our society.

Over the course of ten years, arthritis related work loss has been associated with a 37% drop in income for arthritics - all those without arthritis had a 90% rise in income over the same period of time!

If you...a friend...or a relative has arthritis, it's important to know that early treatment can help sufferers continue with their normal daily lives and remain productive members of the community.

The term "arthritis" is derived from the Greek: "arthron" meaning "joint" and "itis" meaning inflammation. Arthritis is a word that describes over 100 different conditions, some involving inflammation and others not.

Arthritis is not a single disease. It encompasses about 100 different conditions, that affect joints and that pose unique problems for diagnosis and treatment.

Some common types of arthritis include osteoarthritis, rheumatoid arthritis, gout, pseudo-gout, ankylosing spondylitis, polymyalgia rheumatica, psoriatic arthritis, Reiter's disease, systemic lupus erythematosus, and fibromyalgia.

Most types of arthritis involve joint inflammation. Inflammation is the body's natural response to injury or infection.

For an example of inflammation, take a simple scratch...your body automatically releases chemicals that cause fluids to accumulate and white blood cells to gather around the area of the scratch. As your body fights foreign substances and bacteria, inflammation...redness...heat...swelling...and pain occur at the sight of the injury.

In arthritis, unfortunately, this natural defense mechanism goes awry. Elements from the blood designed to fight infection and repair injury attack the body instead.

And, unless this inflammatory process is halted, it will continue to attack the body and cause joint destruction.

So you can begin to see how treatments that just relieve the pain associated with arthritis - but that do not reduce inflammation - may not adequately treat this disease.

Getting proper treatment early on is important...because proper care can help arthritis sufferers lead more active and comfortable lives.

Yet many people with arthritis delay going to a physician. Either they have fear about going to a doctor or they feel that nothing can be done for arthritis. Other reasons include the notion that all arthritis medicines are harmful or arthritis is just a normal part of aging.

Some people try unproven remedies which also delay proper diagnosis and treatment.

Since arthritis may evolve gradually, people often ignore its early warning symptoms or signs. These include persistent pain, tenderness, or swelling in one or more joints...symptoms that should not be dismissed as signs of age.

Other warning symptoms are joint pain and stiffness...especially when they appear in the morning.

Low back pain is one of the earliest symptoms of arthritis. For people over the age of 60, arthritis is the most frequent cause of low back pain.

The activity of arthritis varies unpredictably. Symptoms are cyclic in nature and seem to come and go.

Therefore, it is important to remember that any symptoms or signs of arthritis that last for more than six weeks - no matter how mild - should be checked by a physician. And, if symptoms are severe, then even waiting six weeks might be too long.

The two most common types of arthritis are osteoarthritis and rheumatoid arthritis. Joint inflammation is involved in both.

But, these types of arthritis differ in terms of...age of patients who are affected...the joints involved...the pattern of stiffness...and the potential for disability.

Close to 16 million Americans have osteoarthritis - the most common type of arthritis. Although osteoarthritis can occur at any age, it most often begins in people in their 50's and 60's.

Osteoarthritis or degenerative disc disease is a disorder of cartilage - the gristle that covers the ends of long bones. Cartilage is made of cell called chondrocytes which sit inside a framework made up of collagen and proteoglyens. Under normal conditions, chondrocytes make collagen and proteoglycens - in other works - they make the framework they sit inside. With osteoarthritis, chondrocytes behave abnormally and begin to make destructive enzymes such as collagenasese, stromelysin and others. These enzymes degrade cartilage...these enzymes also attract inflammatory cells which secrete substances called cytokines which cause further inflammation and damage to cartilage, underlying bone, and the joint lining.

This process results in progressive pain, stiffness, and loss of function.

Joint pain and stiffness are the most noticeable symptoms of osteoarthritis. Morning stiffness is usually brief lasting less than 15 minutes. Osteoarthritis usually affects weight bearing areas particularly the neck, low back, hips and knees.

It may also affect the fingers and hands and bony knobs may appear at the finger joints. The base of the thumb may also be affected. The typical pattern of osteoarthritis in the hands involves the distal and proximal interphalangeal (DIP and PIP) joints of the fingers, and the carpometacarpal (CMC) joint of the thumb.

Osteoarthritis is considered to be a degenerative joint disease. Along with inflammation, there is wear and tear on the inside of the joint.

This causes damage to the cartilage (the substance that forms the surface of the joints and works as a shock absorber). As the cartilage wears thin, the underlying bone is damaged. This process results in progressive pain, stiffness, and loss of function.

Osteoarthritis does not need to be disabling and with the proper medical care can be managed easily.

Rheumatoid arthritis is the other most common type of arthritis. It is more common in women and affects 7 million Americans...or one out of every five arthritis patients. It may affect any age group, although onset is most common in middle age.

Rheumatoid arthritis is usually characterized by heat, swelling, and pain in multiple joints in both the right and left sides of the body, including the hands, wrists, elbows, hips, knees, ankles, and feet. Spinal involvement also occurs on occasion.

The typical pattern of rheumatoid arthritis in the hands involves the proximal interphalangeal (PIP) joints, the metacarpal phalangeal (MCP) joints, the wrists, and the elbows.

Unlike osteoarthritis, rheumatoid arthritis can affect the entire body. People with this disease may feel sick all over...tire easily...lose their appetite...and lose weight.

In rheumatoid arthritis, the tissue that surrounds and nourishes the joints is attacked by the body's immune system. The body mistakenly perceives its own tissue as foreign, and it reacts by sending special white blood cells and toxic chemicals called cytokines to destroy the foreign material. (The cytokine abnormalities that cause the damage in rheumatoid arthritis are different from the abnormalities seen in osteoarthritis.) This process of white cell migration and cytokine release damages the joint.

Although we do not know the cause of rheumatoid arthritis, researchers are investigating several possibilities.

Another interesting point about rheumatoid arthritis is that this disease can affect the internal organs including the lungs, skin, blood vessels, spleen, heart, and muscles.

If rheumatoid arthritis is not well controlled it can damage the joints irreversibly and cause serious disability.

To diagnose rheumatoid arthritis, the rheumatologist establishes the presence of joint pain and inflammation lasting at least six weeks and then looks for signs of the course of the disease that are characteristic for rheumatoid arthritis.

There are also blood tests that aid in the diagnosis of rheumatoid arthritis.

Patients with rheumatoid arthritis have a series of flare-ups followed by a period where there are mild or no symptoms. Usually, the pain and disability of rheumatoid arthritis progresses gradually.

Morning stiffness generally lasts longer than half an hour and may last several hours depending on the severity of the condition.

Most forms of arthritis persist for the patient's lifetime. Medication cannot usually reverse the bone and soft tissue damage caused by arthritis.

However, new methods of measuring inflammation and its response to medication and other treatments offer valuable information to physicians...and can help to evaluate the arthritis sufferer's discomfort.

Magnetic resonance imaging is one such technique. This method using the effects that strong magnets have on water molecules to provide exquisite images of the interior of the body. MRI has been used to diagnose and also assess the degree of damage within joints of patients suffering from arthritis. It is also helpful for evaluating the effect of new drugs.

Although there is no cure for arthritis, proper treatment can help tremendously. The goal of arthritis treatment is to relieve the pain and stiffness due to the progressive destruction caused by inflammation, and to maintain or increase freedom of movement.

Among the advancements that have taken place in the medical treatment of arthritis are various disease-modifying medications that not only relieve symptoms but also help slow down the progression of disease.

Other advances include various cartilage sparing drugs, cartilage growing drugs, and also biologic remedies. These drugs act by blocking the destructive effects of enzymes such as metalloproteases in osteoarthritis and cytokines in rheumatoid arthritis. By targeting specific processes, relief of symptoms and healing of damage can take place with presumably fewer side effects.

What can you do if you think you have arthritis?

First, you can consult your doctor. This is important because medical issues are complicated and your doctor, who understands your health needs, can prescribe the best treatment for you.

The type of doctor who can best evaluate arthritis is called a rheumatologist. These are physicians who have completed four years of medical school, three years of internal medicine residency, and three years of rheumatology fellowship.

While arthritis can be a serious disease that can progress and cause disability, science has come up with some new answers for arthritis sufferers. It is now up to the arthritis sufferer to recognize early warning signs and symptoms and to see a rheumatologist. With proper medical care, the course of this crippling disease may change and people can help to be returned to fully active lives - without pain and crippling disability.

What Is Osteoarthritis and What Can Be Done About It?


Osteoarthritis (OA) is the most common form of arthritis and affects approximately 28 million Americans. While it was initially viewed as a "wear and tear" phenomenon, it has become quite clear that it is a disease that is multifactorial in its development.

It is not a benign disease because, in addition to the pain, OA leads to functional disability as well as interference with activities of daily living. Eventually, though, it is the pain that brings the patient to the physician.

The joint is a dynamic structure where anabolic (building) activities are counterbalanced by catabolic (destructive) activities.

With OA, the catabolic activities gradually overtake the anabolic ones. While there are attempts at repair, these attempts are dysfunctional, leading to the formation of bony spurs, called osteophytes.

There are three major risk factors for the development of osteoarthritis. They are genetic (usually a family history is prominent), constitutional (obesity in the case of OA of the knee, and aging), and finally local components (injury, ligamentous laxity, congenital abnormalities).

The development of osteoarthritis starts with an initial injury to cartilage. Cartilage consists of cells called chondrocytes that sit inside a "soup", a matrix, which consists of collagen and proteoglycans.

The injury may trigger an inflammatory response leading to the synthesis of cartilage matrix degrading enzymes, produced by chondrocytes. Over time, the catabolic activities override anabolic activities and abnormal repair mechanisms lead to the formation of osteophytes, while cartilage continues to degrade.

The treatment for osteoarthritis is primarily symptomatic. Analgesics (pain relievers), non-steroidal-anti-inflammatory drugs (NSAIDS), weight loss, exercise, assistive devices such as wedge insoles, braces, canes, walkers, and such. Injection of glucocorticoids and viscosupplements (lubricants derived either from rooster combs or from bacteria) may also be helpful.

Nonetheless, eventually patients will require surgery in the form of joint replacement. Joint replacement surgery has come a long way, but there are still concerns about them. The first is the possibility of a surgical complication such as blood clot or infection. The second issue is the finite lifespan of the prosthesis. They usually last 10 to 15 years but this is a function of activity and joint replacement patients do have restrictions on their activity level. Persistent pain due to particle induced inflammation can also be a problem.

Finally, the chance of faulty prosthetic devices such as the recent Johnson & Johnson metal-on-metal hip debacle, makes the choice of total joint replacement less attractive. In future articles I will discuss an alternative, the use of autologous stem cells to help cartilage regeneration.

Timmermann, PhD, researchers at the University of Arizona, Center for Phytomedicine Research, set up a series of experiments to determine whether (and how) turmeric works as an anti-arthritic. They began by preparing their own extracts from the root of the plant and compared them with commercially available products.

Dr. Funk and her colleagues then tested in animal models a whole extract of turmeric root, only the essential oils, and an oil-depleted extract containing the three major curcuminoids found in the plant root.

Of the three extracts, the one containing the major curcuminoids was most similar in chemical composition to commercially available turmeric dietary supplements. It also was the most effective, completely inhibiting the onset of rheumatoid arthritis.

Dr. Funk states that, "This was the first study that has examined turmeric with the researchers' own prepared, well-defined extracts... the study represents the first documentation of the chemical composition of a curcumin-containing extract tested in a living organism... for anti-arthritic efficacy. It also provides the first evidence of anti-arthritic efficacy of a complex turmeric extract that is analogous in composition to turmeric dietary supplements."

The significance, she explains, is that "translating the results of trials such as these to clinical use depends on accurate information about the chemical content and biological activity of the botanical supplements available for use. This work paves the way for the preclinical and clinical trials needed before turmeric supplements can be recommended for medicinal use in preventing or suppressing rheumatoid arthritis."

Dr. Funk and her colleagues add, "This study also provides the first in vivo documentation of a mechanism of action -- how curcumin-containing extracts protect against arthritis."

"We found that the curcuminoid extract inhibits a transcription factor called NF-KB from being activated in the joint. A transcription factor is a protein that controls when genes are switched on or off. Once the transcription factor NF-KB is activated, or turned on, it binds to genes and enhances production of inflammatory proteins, destructive to the joint.

The finding that curcuminoid extract inhibits activation of NF-KB suggests that turmeric dietary supplements share the same mechanism of action as anti-arthritic pharmaceuticals under development that target NF-KB. It also suggests that turmeric may have a use in other inflammatory disorders, such as asthma, multiple sclerosis and inflammatory bowel disease."

In addition to preventing joint inflammation, Dr. Funk's study shows that the curcuminoid extract blocked the pathway that affects bone resorption. Bone loss associated with osteoporosis in women typically begins before the onset of menopause. Bone loss is also a feature of rheumatoid arthritis.

Author's note: This study is interesting from at least three points of view. First, it confirms the effectiveness of an herbal supplement for adjunctive treatment of rheumatoid arthritis and provides information on a possible mode of action. Second, it gives credence to the use of turmeric in standard dietary supplement form for arthritis. Third, the bone loss inhibiting effect of turmeric is an important one given the magnitude of osteoporosis as a public health issue.
